If my patient has any of the following medical conditions (partial short list), I always prescribe a specialty polarized lens. 

1) Cataracts (all stages) including post-Cataract surgery

2) Dry Eye (all forms)

3) Inflammatory Dry Eye Syndrome (all stages)

4) Diabetes with or without Retinopathy

5) Macular Degeneration (wet or dry)

6) Photophobia
